Let's focus on the ACC Championship game, and in particular the final minutes. UNC down 8 went for an offsides kick, recovered it, only to be called for offsides. Replay showed there wasn't a player within a yard of being offsides. While true, there may have been another violation on the play. Take a look at these two tweets. Assuming the first image is where the players were located at the time of the whistle it appears to have been illegal formation.

Of course the referees called Offsides, not Illegal Formation. In fact the ACC gave this explanation.
